Your First Visit
From the moment you call our dental office, you’ll be asked a series of questions regarding your specific dental needs, ensuring we can customize your appointment to be personalized to you. Once you’re at the dental office, you’ll fill out new patient forms if you have not done so already, then meet with Dr. Moreira directly to have an exam performed. If you have a cleaning scheduled for the same day, you’ll go to have that completed after as well.
Patient Forms
With detailed new patient forms, we can get to know your needs before we even complete a single treatment. You can easily download them from our website and fill them out if you’d like, ensuring that your first appointment is expedited. In them, you’ll find questions regarding your current medical and dental history as well as current medications you are taking. You’ll also have a chance to provide us with information or accommodations we should keep in mind.
Dental Insurance
With a dedicated dental insurance plan, you and your family can get many of your necessary dental treatments done and out of the way. We are happy to be in-network with most major dental carriers and will work with you directly to determine the best method for maximizing your benefits. We’re always happy to help you with filing a claim, work with your insurance provider, and any additional paperwork you need as part of your claim.
